Tell Us Your Story
The National Academy of Medicine defines diagnostic error as the failure to (a) establish an accurate and timely explanation of the patient’s health problem(s) or (b) communicate that explanation to the patient. Simply put, these are diagnoses that are missed altogether, are wrong, are not communicated clearly or should have been made much earlier.
Have you or someone you love been affected by diagnostic error?
At the Society to Improve Diagnosis in Medicine (SIDM) it is our mission to catalyze and lead change to improve diagnosis and eliminate harm from diagnostic error.
We need your help.
Share your story with us by filling out the form below. Stories like yours illustrate the scope and importance of the issue. By sharing your story, you help ensure the voices of patients and their families are heard in diagnostic improvement efforts.
Together, we can find solutions that increase diagnostic accuracy and timeliness and ultimately ensure the best possible health outcomes for patients.
